The report presents outcomes, challenges, lessons learned, and sustainability plans for telehealth grant projects operated by five grantees awarded MHCC telehealth grants the fall of 2015 and summer of 2016. Overall, the grantees’ telehealth projects focused on reducing unnecessary hospital utilization, increasing access to timely and appropriate care, and improving patient satisfaction.
