California’s Santa Clara County Seeks Community Transformation Grant Leadership Coordinator Services

California’s Santa Clara County Public Health Department awarded a contract to provide leadership, coordination, facilitation, and support services for a leadership team comprised of multi-disciplinary and cross sector community members to Creative Mundo Consulting. The role of the leadership team is to advise and leverage resources and capital to support the strategic direction of a community transformation grant (CTG) in a manner that improves collaboration and coordination and maximizes outcomes. The CTG program supports efforts to reduce chronic diseases, promote healthier lifestyles, and reduce health disparities. The contractor will engage the leadership team in the implementation of CTG strategies, support all coordination of the leadership team, be a liaison between the leadership team and the South County Collaborative as well as implement organizational policy and practice change.
