The past seven years have seen an increase in the number of states implementing care coordination initiatives with the goal of improving the quality of patient care while at the same time bending the health care cost curve. State Medicaid programs have been some of the most enthusiastic adopters in care coordination programs with states primarily implementing patient-centered medical homes (PCMHs), health homes, accountable care organizations (ACOs), vertical specialty carve-out health plans, and Medicare-Medicaid dual demonstrations.

This report provides the total number of Medicaid beneficiaries enrolled in care coordination initiatives, an overview of each initiative, the states participating in the initiative, and the number of Medicaid enrollees in that initiative in each state.
