44 State Medicaid Programs Have Care Coordination Programs; Patient-Centered Medical Homes Most Popular Program – OPEN MINDS Releases Guide To Care Coordination For Complex Consumers
Gettysburg, Pa. (June 24, 2018) – A new report from OPEN MINDS found that in 2018, there were 44 unique states participating in 122 distinct care coordination initiatives. These care coordination initiatives included:
- Accountable care organizations (ACOs)
- Patient-centered medical homes (PCMH)
- Patient Protection and Affordable Care Act (PPACA) health homes
- Specialty consumer health plans
- Dual eligible demonstrations
- Managed long-term services and supports (MLTSS)
- Certified Community Behavioral Health Clinics (CCBHC) demonstration
Among these seven programs, patient-centered medical homes were the most adopted initiatives, with 32 states participating, and CCBHCs were the least adopted initiative, with only eight states participating. On average, states adopted three to four of the listed initiatives. The six states that have not adopted any of these care coordination initiatives are Alaska, Indiana, Kentucky, Louisiana, Mississippi, and North Dakota.
