This paper explores the landscape of private equity backed organizations acquisition of I/DD providers. With an I/DD market increasingly shaped by external challenges of managed care reimbursement, the implementation of assistive technology devices to deliver care, and competition from the consolidation of service delivery and service delivery organizations, more and more independent provider organizations are faced with the strategic challenge of sustainability. Internal challenges include critical staffing shortages, under funded back office support functions such as payroll and accounting, limitations on IT resources due to budget constraints, and increased competition from larger provider organizations able to provide services at less cost and with greater scope. For many independent provider organizations aligning with another organization may be the best option to tackle these challenges and ensure future sustainability of services.

Private equity backed organizations are stepping in to buy provider organizations through a variety of arrangements. The resulting larger provider organizations are realizing greater purchasing and political power resulting in not only a profit but provider organizations that are able to meet marketplace challenges, innovate and continue to deliver high quality services.
