Colorado Medicaid Managed Care Contracts
The Colorado's Medicaid managed care program is called the Accountable Care Collaborative (ACC). On July 1, 2018, Colorado transitioned to an integrated care model that uses Regional Accountable Entities (RAEs) as the single entity to coordinate both physical and behavioral health services.
The state selected five RAEs, each of which serves one or more of the state’s seven regions. Individuals are attributed to a primary care provider (PCP). The location of the PCP determines their RAE attribution. The five health plans associated with seven regions are available for download:
