How can technology tools improve population health and care management?
Recently, the National Institute of Mental Health cited that one in six patients had a prevalence of Any Mental Illness (AMI). In 2016, this number represented 44.7 million adults 18 or older which represented 18% of all U.S. adults. Patients with mental illness will have increased cost and utilization across any given patient population. Technology solutions can assist organizations by providing the guidance needed to deliver the right care at the right time, while reducing costs and delivering outstanding financial results.
This white paper studied AMI populations, and observed the differences between patients who were involved in a formalized care management program, to determine how to better serve patients while reducing care costs. TopazIS conducted in-depth research and goes into detail outlining different patient populations, behavioral health cohort management, ER visit reduction opportunities, and how to operationalize behavioral health care management by utilizing TopazIS’ cohort builder and population health management tools.
