This document is a concept paper released in December 2017, by the Hawai‘i Department of Human Services (DHS) Med-QUEST Division (MQD). MQD is preparing to launch a Medicaid managed care initiative to integrate services to address health-related social needs and individual behaviors that influence health and well-being.
