Rite Aid Launches Pharmacy-Based Chronic Disease Management, Partnering With ACOs & Medical Homes

On March 11, 2014, retail pharmacy chain Rite Aid launched the Rite Aid Health Alliance, in which Rite Aid pharmacists and in-store care coaches work with physicians to provide community-based health management services to individuals with one or more chronic health conditions. Rite Aid partnered with a Medicare accountable care organization (ACO) and a Medicaid health home provider organization to pilot the services.
