HealthPartners Addiction Treatment P4P Program With Hazelden Cut Readmission Rates

A HealthPartners pay-for-performance (P4P) program for addiction treatment at residential facilities operated by Hazelden Betty Ford Foundation (HBFF) resulted in the reduction of one-year readmission rates by 0.2% percentage points between 2013 and 2014. One-year readmission rates dropped from 6.7% in 2013 to 6.5% in 2014. Further, the percentage who attended at least three 12-step meetings per week during the first six weeks post-discharge grew from 39% in 2013 to 42% in 2014.
