Nurse practitioner-led medication reconciliation of those being admitted to a skilled nursing facility (SNF) reduced hospital readmissions within 30 days by about 29.7%. The hospital readmission rate was 19.2% prior to nurse practitioner-led medication reconciliation, and 13.5% following the reconciliation efforts. The national average hospital readmission rate is 21.1%. SNFs are penalized for hospital readmissions within 30 days, and medication errors often precipitate hospital returns.
