The New York Office for People With Developmental Disabilities (OPWDD) in conjunction with the New York Department of Health (DOH) plans to launch care coordination organization health homes (CCO/HHs) for consumers with developmental disabilities. The CCO/HHs will be responsible for coordinating physical health, behavioral health, long-term services and supports.
