Michigan Medicaid is planning to launch the MI Care Team primary care health home pilot program in April 2016. The state released an invitation to bid (ITB 043116B0006411) on November 6, 2015, with proposals due from Federally Qualified Health Centers (FQHCs) and Tribal Health Centers (THC) by December 8, 2015.
