High Intensity Of Care Management & Community Health Worker Intervention Associated With Higher Health Care Utilization
High intensity care management (CM) coupled with a high intensity community health worker (CHW) intervention was associated with higher use of hospital care and readmissions. In a recent grant-funded study, those who received a high CM–high CHW intervention had a 44% greater chance of hospitalization, and a 120% higher chance of 30-day hospital readmission compared to a reference group that received low intensity CM and low intensity CHW.
