Delaware Launching New Care Coordination System For Behavioral Health Consumers; Issues RFP For Care Coordination Platform
The Delaware Department of Health and Social Services (DHSS) is preparing to launch a new comprehensive and statewide care coordination system that connects behavioral health consumers served by multiple DHSS agencies with health and social services provider organizations. To prepare for the launch, the state released a request for proposals (RFPs) on June 24, 2020 (RFP HSS-20-041) for a comprehensive care coordination platform.
The platform is intended to help provider organizations identify, track, and better assist consumers by removing the barriers to care posed by behavioral, economic, and social determinants of health. Responses are due by August 12 . . .
