Continuity Of Care Reduced Drug-Drug Interactions
Greater efforts to ensure continuity of care (COC) for people with multiple comorbid health conditions is linked to lower risk of drug-drug interactions (DDI). A drug-drug interaction occurs when a person is prescribed two or more drugs simultaneously that are contraindicated for use at the same time because the combination creates side effects or reduces the effectiveness of one or both drugs.
