California Medicaid Developing New ‘Long-Term Care At Home’ Benefit
On May 22, 2020, the California Department of Health Care Services (DHCS) announced it was developing a potential new Medicaid “Long-Term Care at Home” benefit. The model of care will include four elements: individual, person-centered assessment; transition services; care management; and home- and community-based services (HCBS). The benefit will include services to support home care for individuals transferring to home from hospitals and skilled nursing facilities (SNF). The services will be intended to help prevent a SNF stay or SNF readmission among at-risk consumers. DHCS intends to implement the model by early 2021, assuming timely approval . . .
