The California Department of Health Care Services (DHCS) is preparing to launch a three-year Medicaid Medically Tailored Meals Pilot Program after January 1, 2018. The pilot will involve Medi-Cal (California Medicaid) beneficiaries with selected chronic health conditions who live in the following counties: Alameda, Los Angeles, Marin, San Diego, San Francisco, Santa Clara, and Sonoma.
