Wisconsin Implementing Foster Care Medical Home Program In Six Counties

On December 12, 2012, the Wisconsin Department of Health Services (DHS) released a solicitation seeking applications from health systems to become certified as Medicaid Foster Care Medical Home (FCMH) provider organizations for Milwaukee, Waukesha, Racine, Kenosha, Washington, and Ozaukee counties. Wisconsin has roughly 6,000 children in out-of-home placements who receive Medicaid services; 2,500 are in the six counties.
