Minnesota North Memorial Health System Sends Paramedics On House Calls To Reduce Repeat Emergency Visits
To reduce repeat emergency department visits, the North Memorial Health System in Minnesota, implemented a program that sends community paramedics to patients' homes. The program focuses on home visits to patients who visited the emergency department nine or more times in the previous year.
