The Path To Building A Consumer-Centric Link Between Hospitals & The Community
Last month my colleagues Sarah Threnhauser and Richard Maye discussed the continued problem of high hospital readmission rates – Care Coordination – Fixing Those ‘Silos’ That Cause Readmissions. This is an issue where we're seeing very little movement (the rates moved from 21.5% in 2007 to 17.8% in 2015 (see 3.7% Drop In Hospital Readmission Rates Between 2007 & 2015; Drop Attributed to Medicare Readmission Penalties) – which will keep this a front-and-center issue for health plan, hospital, and community-based provider organizations alike.
