It’s 2015! Where Are We With Health Homes?
It has been three years since the first health homes were first launched in Missouri, and then shortly followed by Rhode Island. The health home construct is a model of specialty medical homes that was imbedded in the Patient Protection and Affordable Care Act (PPACA) – an option for state Medicaid plans to try intensive care management for consumers with complex conditions. So what has happened since then?
We now have sixteen states that have health homes up and running – Alabama, Idaho, Iowa, Kansas, Maine, Maryland, Missouri, New York, North Carolina, Oklahoma, Oregon, Rhode Island, South Dakota, Vermont, Washington, and Wisconsin . . .
