By Athena Mandros It has been three years since the first health homes were first launched in Missouri, and then shortly followed by Rhode Island. The health home construct is a model of specialty medical homes that was imbedded in the Patient Protection and Affordable Care Act (PPACA) – an option for state Medicaid plans to try intensive care management for consumers with complex conditions.  So what has happened since then?
