This month, we reviewed the stories that were most read by OPEN MINDS Circle readers during the first half of the year. One common thread — collaborative care models and integrated delivery systems are high on the list. These were a few of the stories that were the most read by our readers:
But the landscape for collaborative care and integrated service delivery system models is evolving. A number of states are moving forward with the implementation of health homes. And, in addition to the launch of Medicare ACOs (CMS Releases Final ACO Regulations for Medicare Shared Savings Program ), a number of state Medicaid plans have announced plans to create Medicaid ACO programs (Emerging Medicaid Accountable Care Organizations: The Role of Managed Care ). And, the early adopters of health homes, both Missouri and Rhode Island, are beginning to track outcomes from fully implemented systems (for an inside look at Missouri and Rhode Island’s health home systems, check out these presentations from the 2012 OPEN MINDS Planning & Innovation Institute – Missouri’s Primary Care And CMHC Health Homes and Rhode Island’s Program For Medicaid Health Homes ).
At the same time, efforts to standardize the concepts of medical home and health home are underway. Both CARF (see CARF Launches Health Home Accreditation ) and the Joint Commission (The Joint Commission Primary Care Medical Home Model premium member) have launched accreditation initiatives in this area.
