An important part of addressing social determinants of health (SDH) is shifting to a "prevention" mindset. This doesn't mean that you can address all of a consumer's needs through preventative care – but focusing on community-based interventions and services that consider consumers as a whole person can help organizations to address issues before they become too destructive to a consumer's health.
