URAC Releases Standards For Patient Centered Medical Home Certification
URAC, an independent accreditor that develops evidence-based measures and standards for the health care industry, has released its proprietary standards for the organization’s Patient Centered Medical Home (PCMH) program in an effort to help primary care practices nationwide implement best practices for providing value-based, quality medical services. The PCMH program identifies and addresses essential standard elements needed for sustainable transformation from a physician practice or health care clinic into a patient centered medical home, including:
- Enhancing access to primary care,
- Improving delivery of preventive services,
- Helping patients make healthy lifestyle choices . . .
