Eight New Hampshire health care organizations, including the hospitals of GraniteOne Health and several members of North Country Community Care Organization (NCCCO), have formed New Hampshire Value Care ACO, LLC (NH Value Care) to increase quality and value for New Hampshire Medicare consumers.
NH Value Care was recently approved by the Centers for Medicare & Medicaid Services (CMS) as an Accountable Care Organization (ACO). An ACO encourages members to look at care on a more coordinated basis, identifying best practices and reducing redundancies that help to increase quality and reduce cost. The goal is to provide consumers with integrated, coordinated, and consistent care.
ACOs fall under the Medicare Shared Savings Program, which was created by the Affordable Care Act. Members share both financial and medical responsibility of providing care to its total consumer population. The ACO is accountable to several CMS measures of quality and utilization. When an ACO successfully meets these measures—providing care that is high-quality and cost effective—they are eligible for payments (shared savings) from CMS. ACOs tend to place a strong emphasis on primary care, population health, and prevention.
The members of NH Value Care include: GraniteOne Health, including Catholic Medical Center, Huggins Hospital and Monadnock Community Hospital as well as certain members of North Country Community Care Organization, LLC, including Ammonoosuc Community Health Services, Androscoggin Valley Hospital, Littleton Regional Hospital, Upper Connecticut Valley Hospital and Weeks Medical Center. Indian Stream Health Center in Colebrook is expected to join on January 1, 2020.
NH Value Care, LLC is a limited liability corporation that was set up in 2018 for the express purpose of participating as an Accountable Care Organization in the Medicare Shared Savings Program in partnership with other health care organizations. NH Value Care includes eight New Hampshire based organizations. Collectively the eight organizations employ over professionals and provide services to approximately 20,000 Medicare beneficiaries attributed to the ACO.
