Cantex Continuing Care Network and Diakon Lutheran Social Ministries announced in August 2019, along with Netsmart, they were launching the Netsmart Referral Manager. The system allows more efficiency and reflects a growing need to better manage referrals. The automated solution eliminates phone calls, faxes, and manual data entry. With the Referral Manager, Cantex and Diakon reported the time to process qualified referrals has decreased by 73%.
According to Mike Valentine, chief executive officer of Netsmart, providing a clinical consumer-view that allows for real-time integration is crucial to the referral decisionmaking process. Additionally, according to Deanna Ziemba, senior vice president of Diakon Lutheran Social Ministries, clinical professionals and staff are able to quickly and efficiently complete intake forms and notes, significantly reducing the referral process time and the need for unnecessary documentation.
Cantex Continuing Care Network was formed in 1978. With 41 successful years of business, the network operates 35 nursing facilities with a total of 4,158 licensed beds. The nursing facilities are located in Texas and New Mexico, are certified under the federal Medicare program and participate in the state’s Medicaid program. Rehabilitation is provided to Cantex consumers by the in-house therapy division. The post-acute network is fully integrated and promotes a seamless experience for the consumers in those markets serviced by the home and community-based services division including TheraCare Home Health and Elysian Hospice.
Through Diakon Lutheran Social Ministries and Diakon Child, Family & Community Ministries, a continuum of senior lifestyle and health care services, including Continuing Care Retirement Communities, and a range of programs for children, youth, families, and adult individuals of all ages and entire communities in Pennsylvania and Maryland. Annually, the programs directly serve between 60,000 and 70,000 people. Working in nearly 30 service centers and senior living and housing communities, approximately 2,000 Diakon staff members daily bring hope, hospitality, and healing to thousands of people.
