Healthfirst, New York’s largest non-profit health insurer, and Landmark Health (Landmark), an industry leader of in-home medical care, will work together to offer convenient, in-home medical care and coordination by clinical professionals, nurse practitioners, and physician assistants to nearly 7,500 Healthfirst Medicare Advantage members with complex, chronic conditions. According to Thomas Meixner, senior vice president of Delivery System Innovation at Healthfirst, offering services through Landmark will allow for an increase in access to quality care for the members that need it the most.
For consumers living in the boroughs of New York City, getting to the doctor regularly can be challenging. Consumers with complex heath issues often have limited mobility and need to rely on a loved one to bring them to office-based appointments or urgent care. Landmark’s medical professionals are available 24/7 and bring care to members in their homes through routine and urgent house calls. Pharmacists, dietitians, nurse care managers, social workers, and behavioral health specialists are all part of the Landmark care team, available to members both in home and over the phone. Landmark provides supplemental care, in collaboration with consumers’ existing doctors and specialist, to improve access to services.
The Landmark program is voluntary and offered at no cost to eligible Healthfirst Medicare Advantage members. Members with multiple chronic conditions, such as diabetes, coronary heart disease, and chronic kidney disease, along with social or behavioral needs, may qualify for the Landmark program.
