Aspen RxHealth announced that Humana Inc., a leading health and well-being company, has chosen the company’s app-based platform to connect pharmacists with patients to deliver clinical services, including medication therapy management (MTM). Aspen RxHealth’s innovative approach to establishing the patient-pharmacist relationship is built on the model of crowdsourcing and enabling a two-sided marketplace.
Aspen RxHealth will leverage their community of pharmacists to contact Humana Medicare Advantage members and review medications, provide education, and answer questions about their therapy. Humana launched this program in Tampa, Florida, with a potential expansion into other markets later this year.
The pharmacists in the Aspen RxHealth community empower patients with the information and action plans they need in an effort to positively impact their health care goals. Improved interoperability and increased coordinated care allow the pharmacist to better ‘plug in’ and serve as a cornerstone of the patient’s extended care team. According to Scott Greenwell, president of Humana Pharmacy Solutions, the collaboration with Aspen RxHealth will allow members to directly contact pharmacists about medication plans and treatment goals, ultimately facilitating consumer engagement and medication management.
Aspen RxHealth develops technology that connects a community of pharmacists with patients. The company removes the barriers that currently exist for pharmacists to engage in direct care services. Aspen RxHealth’s community of pharmacists empowers patients with the information and action plans they need to achieve optimal outcomes from their medications.
Humana Inc. is committed to helping millions of medical and specialty members achieve their best health. The successful history in care delivery and health plan administration is helping create a new kind of integrated care with the power to improve health and well-being and lower costs. The efforts are leading to a better quality of life for people with Medicare, families, individuals, military service personnel, and communities at large.
