Ascension Joins Project To Create Age-Friendly Health Systems
The John A. Hartford Foundation (the foundation) has announced that Ascension will be the latest health system to participate in its prototype development of an age-friendly health system. The Institute for Healthcare Improvement and the American Hospital Association have also formed a partnership with the foundation in working towards the initiative.
Also joining in the three year initiative to develop and test the new model of care for older adults are Arundel Health System, Kaiser Permanente, Providence St. Joseph Health, and Trinity Health. Ascension's participation will allow the project's potential reach to increase to 40 states. The . . .
