Health homes are a population-based integrated care service delivery model (not a physical space) targeting people with chronic conditions. The health home model is an outgrowth of the medical home model that became a new Medicaid state option under health care reform legislation (Section 2703 of the Patient Protection and Affordable Care Act, or PPACA). The health home model is independent of delivery system.
