The health and human service delivery system has changed steadily in the last several decades—and a huge part of that change has been the shift from institutional solutions to tech-enabled community-based solutions. What does this shift mean for health plans and provider organizations interested in serving consumers "where they live?" First, there will be a bigger demand for community-based programs, particularly those that emphasize person-centered planning and consumer choice.
