Magellan Tops Behavioral Health Management Market Share: Enrollment Growth Largest In EAP Sector Over Past Decade
In 2011, Magellan Health Services, Inc. held the greatest market share among all managed behavioral health, employee assistance, and disease management programs in the U.S. Total enrollment among these organizations increased by 111% since 2002, and of that group, the employee assistance sector showed the greatest amount of growth, with an enrollment increase of 200% since 2002.
Magellan Health Services dominates the managed behavioral health and employee assistance program (EAP) market, capturing more than a 16% market share in 2011. This compares to 2002, when Magellan represented around 30% of total managed behavioral health and EAP enrollment. The three vendors with the highest enrollment figures are currently Magellan Health Services, OptumHealth, and ValueOptions. In 2002, the same three showed the highest enrollment figures, but OptumHealth (as United Behavioral Health) ranked third behind ValueOptions. Together, these "top three" vendors represent around 35% of the 2011 market, compared to almost 51% of the market in 2002.
Over the course of the past nine years, enrollment in
the EAP sector has increased by over 200%. Of the
total EAP enrollment, integrated EAP/work-life
assistance programs make up 44%; work/life
assistance programs make up 20%; EAP-only programs make up 18%;
integrated EAP/managed behavioral health ASOs make up 9%; and risk-based integrated EAP/managed
behavioral health programs make up 9%.

In 2011 there was a greater balance among the "top ten" vendors with the highest enrollments than was seen in 2002. But when taken as a whole, this "top ten" group still controls about the same percent of the entire market; in 2002, the "top ten" represented a little over 75% of the market and in 2011, they account for 73% of the market. While the market share controlled by the top ten organizations remains virtually the same, some significant shifts have occurred since 2002. Three new players joined the ranks of the "top ten" in 2011: Aetna Behavioral Health; Workplace Benefits, LLC; and LifeCare. The following table captures some of that shift:

These were the findings of an in-depth market analysis, conducted by OPEN MINDS, of U.S. managed behavioral health, employee assistance, and disease management programs. To determine enrollment, OPEN MINDS surveyed over 360 managed behavioral health organizations, EAP, and specialty disease management programs. Over 88% of this year’s total enrollment was self-reported by surveyed organizations. Because enrollment in these program types is not exclusive, individuals could theoretically be enrolled in all three program types.
