Multiple Chronic Conditions: A Strategic Framework from the U.S. Department of
Health & Human Services
On December 14, 2010, to reduce the problem of fragmented care, the U.S.
Department of Health and Human Services, Office of the Assistant Secretary for
Health released the framework on that reflects the efforts and input of agencies
within HHS and multiple private sector stakeholders. The focus is on fostering
change within the system; providing more information and better tools to help
health professionals and their patients learn to better coordinate and manage
care.
