Whole System Demonstrator Programme: Headline Findings—December 2011

The British Department of Health started the program in May 2008 as a large randomized trial involving 6,191 patients and 238 primary care practices in three sites. The Department of Health sought to test the ability of remote monitoring technology to help people with chronic health conditions avoid hospitalization. Half of the patients, about 3,031 people had diabetes, heart failure, or chronic obstructive pulmonary disease. The early outcomes are based on 12 months of data.
