End-of-Life Care: Key Components Provided by Programs in Four States
The research was conducted by the Government
Accountability Office (GAO) to respond to a request from U.S. Senator Ron
Wyden to identify examples of programs that provide key components of
end-of-life care. From June 2005 to August 2007, GAO reviewed studies by
the Institute of Medicine and the federal Agency for Healthcare Research
and Quality. In addition, GAO conducted site visits in four states
Arizona, Florida, Oregon, and Wisconsin.
Four programs rely on care management techniques to incorporate six key
components of quality end-of-life care. In these programs, some care
providers relied on care management through interdisciplinary teams and
others used a single case manager. Patients were encouraged to discuss
the types of services they might need. The six components are: care
management to coordinate service delivery; supportive services for people
outside institutional settings; pain and symptom management; family and
caregiver support; communication among the individuals, families, and
program staff; and help with advance care planning. with high usage of
end-of-life services.
