ShareProposals for Community Long-Term Care Services Coordinated Care Pilot Programs

The State of Colorado Department of Health Care Policy and Financing (HCPF) seeks to create a coordinated long-term care delivery system for the State Medicare program to provide improved structure and quality of provider operations and enhance the quality of life for program participants. The request for proposals was issued November 30, 2006. The HCPF is especially interested in proposals that coordinate Medicaid and non-Medicaid services to test and evaluate the integration of financing streams.

Each proposal considered will provide services in at least two rural communities and three urban communities for a three-year term. Services must address the needs of consumers with behavioral issues, dementia, mental illness, or brain injury in a variety of settings to ensure consumer choice.
