Prepaid Mental Health Plan, AHCA Area 11

The contract resulting from this solicitation will be a fixed capitation rate contract payable from Medicaid funds appropriated in the FY 2005-2006 General Appropriations Act. The capitation rates payable under this contract may be recomputed annually.

The contract is anticipated to begin on May 1, 2006, or upon execution by the Agency and the successful vendor, whichever date is later, and remain in effect through April 30, 2009. In accordance with Section 287.057(14), Florida Statutes, the contract resulting from this solicitation may be renewed for a period that may not exceed three (3) years or the term of the original contract whichever period is longer. Renewal of the contract shall be in writing and subject to the same terms and conditions set forth in the original contract. A renewal contract may not include any compensation for costs associated with the renewal. Renewals are contingent upon satisfactory performance evaluations by the Agency, are subject to the availability of funds, and optional to the Agency.
