 Avoidable Hospitalizations in Pennsylvania
Timely and effective primary care may reduce the likelihood of hospitalization for certain conditions including pneumonia, diabetes, asthma, and hypertension - often referred to as ambulatory care sensitive conditions. Because early intervention in the outpatient setting can frequently prevent complications or more severe disease, hospitalizations for these conditions may be
preventable.
