 Indiana FSSA Request for Service (RFS-08-14) for
Bureau of Quality Improvement Services
Indiana Department of Administration made a request for proposals (RFP)
on behalf of the Family and Social Services Administration. The Request
for Service (RFS-08-14) was for Bureau of Quality Improvement Services
for Indiana residents with developmental disabilities. The vendor would

The term of the contract shall be for a period
of two years from the date of contract execution. There may be two
one-year renewals for a total of four years at the States option.
