CMS Letter to State Medicaid Directors & Health Officials About Health Homes for Enrollees With Chronic Conditions

The Centers for Medicare and Medicaid Services (CMS) released preliminary guidance about how states can provide health homes for Medicaid beneficiaries with chronic health conditions. The guidance includes CMS expectations for implementation of the home health model of service delivery and initial guidance on aspects of the home health provision in the Patient Protection and Affordable Care Act (ACA) of 2010.
