This spreadsheet, created by the Michigan Department of Community
Health Services, provides a listing of the CPT/HCPCS codes and
reimbursement rates for a variety of medical services that can be
provided through tele-health technology as well as in an office
setting. Psychiatric rates include individual counseling, medication
management, and evaluation. Counseling rates vary depending on the
length of the appointment, the time of day. Rates for service on
weekends and evenings are higher than those for services delivered
during standard working hours.
