ShareRecertification Inspection of William F. Green State Veterans Home

The Alabama Department of Health (DOH) conducted a scheduled recertification inspection and at the same time investigated complaints related to two patient deaths at the William F. Green State Veterans Home. The last state recertification inspection of the William F. Green facility took place in August 2003. The facility was last inspected in November 2006 by the federal Department of Veterans Affairs.

The DOH reviewers noted that they heard reports that some staff and patient or family interactions involved mental or verbal abuse. Some staff failed to follow correct nutrition or medication practices. Some residents experienced a lack of attention. One of the five wings of the facility had overly hot water, which presented safety concerns, although no residents were harmed. Ants were observed near garbage dump areas.
