The Pennsylvania Nursing Home Transition project has been expanded
statewide.
Each county has a collaborative partner in the nursing home
transition project. This document lists the partner for each county,
except for Lycoming and Columbia counties, which at the time this
list was published had not selected a collaborative partner.
