Missouri Medicaid CMHC Health Homes State Plan Amendment
The State of Missouri amended its Medicaid plan to offer Health Home services for individuals with chronic conditions.
Those with serious mental illness or addiction disorders will receive care in
Health Homes operated by community mental health centers (CMHC). The amendment was approved on October 20,
2011 and went into effect as of January 1, 2012.
