ShareNew York State OCFS & DOH Approved Rates for B2H Waiver Program

This rate table describes the services and rates approved for the "Bridges to Health" (B2H) program operated by the New York Office of  Child and Family Services (OCFS) and the Department of Health (DOH) that begins in January 2008. The program provides supportive services to children with serious disabilities (developmental, mental health, or medical) who have been placed in the foster care system.

B2H services include skill training, habilitation, pre-vocational training, supported employment. The program covers the cost of specialized adaptive modifications to homes and assistive equipment. The children and families will also be eligible to receive crisis and respite services and advocacy for the children's participation in school and other community activities.

The rates are divided by service type, disability type, and the child's location; for example, health care integration may be billed for the following monthly rates: service providers in upstate New York will be paid $1,854. In downstate regions, the rate is $1,977. Respite care rates are slightly higher for medically fragile children.
