ShareA New York City Department of Investigation Examination of 11 Child Fatalities & One Near Fatality

The report was conducted by the New York City Department of Investigation (DOI) and the Administration for Children's Services (ACS) at the request of New York Mayor Michael Bloomberg. The DOI reviewed ACS actions in the cases of 11 children who died and one who nearly died between October 2005 and July 2006 after claims of neglect or abuse had been reported to ACS.

In all but one of the cases, DOI labeled the ACS investigations as substantially inadequate and incomplete. The report noted that a key problem in the child deaths was that ACS caseworkers often accepted the parents word that nothing was wrong, failed to interview witnesses, and failed to examine records that could have revealed patterns of abuse.
