HEAL NY Phase 17 Request for Grant Applications: Expanding Care Coordination Through the Use of Interoperable Health IT (RGA# 1006090831)

On September 10, 2010, $109 million in health information technology grants were awarded through the Health Care Efficiency and Affordability Law of New York (HEAL NY), primarily for the implementation of medical homes for individuals with mental illnesses. The New York State Department of Health and the Dormitory authority of the State of New York awarded the two-year state grants to 11 health care organizations for projects to coordinate health care via health information technology. Most of the funding went to organizations that are planning to implement a Patient-Centered Medical Home model of care for people with serious, chronic, mental health disorders, such as major depression and schizophrenia. Several organizations will also address chronic illnesses such as diabetes and other metabolic disorders.
