Uniform Application for the Community Mental Health Services Block Grant & Substance Abuse & Prevention Treatment Block Grant FY 2012–2013 Application Guidance & Instructions

For fiscal year 2012-2013, states seeking community mental health grants and substance abuse grants from the Substance Abuse and Mental Health Administration (SAMHSA) must submit their plans for priority populations, services to be offered, performance metrics to assess the services offered; the plans will be used to develop state-specific performance dashboards. SAMHSA is creating national dashboard indicators during fiscal year 2011. The planning requirements and dashboards are part of SAMHSA’s efforts to help states prepare for the 2014 Medicaid expansion, mandated by the Patient Protection and Affordable Care Act of 2010 (ACA).
