ShareQuality in Integrated LTC Systems

Walter N. Leutz, Ph.D., M.S.W., an associate professor in the Heller School for Social Policy and Management at Brandeis University, gave this presentation during The Hilltop Institute Symposium on Thursday, June 11, 2009. Dr. Leutz presents three levels of integration: linkage, coordination, and full integration. He also addressed quality concerns, quality measures, and potential solutions in the following key areas of need: linkages across systems, communication, information, relationships, and transitions.
